Managers at the Center should check out the new separation form on the Hutch Internet site at http://www.fhcrc.org/admin/hr/pppm/p0312.htm.
Hard copies of new forms will be sent to managers and payroll administrators with the June paychecks on Friday, June 29, so they should toss out old forms and make room for the new.
The new design of the form serves as a reminder to managers to turn in separation forms quickly when a staff member leaves the Center.
"Timely processing is critical to the Center," said Nicole Reed, new-hire coordinator. "The Center is exposed to losses if the forms aren't submitted in a timely manner."
Reed listed these examples of loss to the Center in such circumstances:
A separation form should be completed once a supervisor is notified of a staff member's resignation, Reed said.
This allows time to set-up an exit interview, check for outstanding Center property and settle any issues prior to the staff member's departure.
"We kindly ask that separation paperwork be completed and returned to us within 24 hours following the staff member's last day worked," Reed said.
Time & Effort cards should be completed and returned with the separation form, she said. Vacation time may not be used to extend a staff member's termination date.
