If you are interested in joining the CARB Study, the first step is to complete the Screening Questionnaire.
You may get a copy by email at carbs@fhcrc.org or call the CARB Study phone line at (206) 667-2547. Please leave us your full name and mailing address. We will send you the CARB Study Information Booklet along with the screening questionnaire and a stamped, addressed envelope to return to us.
